  1. Overview of formation and goals of Collaborative

Pumps:  WIC has purchased some Hygeia pumps.  MMC & MCH trialing Ameda

Cindy Smith rents/sells Hygeia   Currently MMC, Mercy & Bettina rent/sell Medela. St. Mary’s rents Ameda.

  1. Linda has been doing a survey of  charges for  inpt. and opt. lactation services in hospitals and PPLCs.  Everyone finds that some moms are not accessing needed help because of the cost and lack of insurance reimbursement.  Mercy also feels they do not have the hours necessary to provide opt. visits.

-Sheila will check what information ILCA offers for insurance reimbursement.
-Cindy and Bettina will check what PPLC resources offer.
-Hospital LCs will check with their billing departments to see how they are billing and what Maine Care and  insurance companies are reimbursing.
-Carole will ask about insurance reimbursement in MA
Medela’s website offers families the following suggestions on dealing with insurance companies http://www.medelabreastfeedingus.com/breastfeeding-insurance

Medela Nipple Shield Handout

Carole shared about the MA conference she and Gail Martel attended recently. Specifically about nursing practices at night that promote breastfeeding and help parents prepare for caring day and night for their babies after discharge.

USBC link is on MELCA website, www.melcaonline.org click and see the webinars they offer.

  1. Kathryn Landon Malone PNP at True North does group well baby care and feels she is seeing problems from overuse of nipple shields and lack of follow-up. Mercy provides Medela’s handout instructions and informed consent (is there a link for this?).  Hospital LCs do FU calls, but many moms don’t call back.  Support group is a great way to offer free wt. checks and quick check ins.  If more help is needed, moms should be encouraged to make an appt.  With use of a nipple shield, assessing milk transfer, wt. gain and milk supply may be critical to successful breastfeeding. What to do if moms don’t follow up? Some pedis do 2 wk. wt. checks.  Consider coordinating wt. checks with VNA and/or pedi. at discharge or schedule a FU LC opt. appt. 48 hrs after discharge.
  1. Discussed posterior-tongue tie resources,  Dr. Damien has been very helpful as a resource. www.kiddsteeth.com  has info on tongue-tie (good pics and articles) Article “Tongue Tie and Frenotomy in the Breastfeeding Newborn” by Isabella Knox, Vol 11 No 9 September 2010 from www.neoreviews.org  (AAP journal).
  1. Skin to skin video shows 9 steps babies go through after birth to first latch. Pam Tozier shows it to staff.
